1. Be Sure You Are Registered to Vote! If you are not, please go to the voter registration section first for more information.
2. Request an Application for Absentee Ballot by Mail Download the Application for Absentee Ballot (EB-121). Complete the application form and mail it to the village clerk's office at 500 Madison Ave., Sullivan, WI 53178. Or you may drop it off in person to the clerk’s office at that same address. The application must be received by the clerk no later than 4:30 PM on the Friday before the election in order for an absentee ballot to be mailed to you. OR 3. Request in Person at the Village Clerk's Office You may complete an application form at the clerk’s office up until 4:30 PM on the day before the election. You may then vote in person at the clerk’s office.
Deadline For Return: The completed ballot must be received by the village clerk no later than the day of the election so that it can be delivered to the polling location by 8 PM on Election Day.
